THOUSAND ISLAND PARK CORPORATION

CONTRACTOR REGISTRATION AND LICENSING FORM

All contractors and sub-contractors working in Thousand Island Park must be registered with the Corporation Office. If a contractor or sub/contractor is found working in TI Park without a valid T. I. Park Corporation license, a Stop Work Order will be issued. The registration and licensing procedures are listed below.

1. Registration Form
   Complete and submit this form to the Corporation Office

2. Contractor License Fee
   The fee is currently $250 per year for non-resident contractors and $150 per year for resident contractors whose name is listed on a current T.I. Park Cottage Site Lease.

3. Certificate(s) of Insurance
   Submit a current liability insurance certificate & current worker’s compensation certificate if applicable. Insurance must have limits no less than $1,000,000 per occurrence and $2,000,000 aggregate. Upon renewal, it is the contractor’s responsibility to ensure the Corporation Office has the renewal on file. Email: mgagne@tiparkcorp.com
